Personal Fat Threshold

The personal fat threshold is the idea that each person has a limit of fat storage — once exceeded, fat spills into organs and can trigger type 2 diabetes.

Plain English

Patient-Friendly Explanation

The personal fat threshold is a concept that explains why some people develop type 2 diabetes at a fairly modest weight while others do not at a much higher one. The idea is that everyone has a limit to how much fat their body can safely store under the skin; once that personal limit is reached, extra fat starts building up where it does harm — inside the liver and the pancreas. When the pancreas accumulates enough fat, it can no longer produce insulin properly, and blood sugar rises into the diabetes range. Lose enough weight to drop back below that threshold, and the organs can clear the fat and diabetes can go into remission.

Medical

Clinical Definition

The personal fat threshold concept, articulated by Professor Roy Taylor and the Newcastle group, proposes that type 2 diabetes develops when an individual’s capacity for subcutaneous adipose storage is exceeded, forcing ectopic fat deposition in the liver and pancreas (the twin-cycle hypothesis). Because the threshold is genetically and ethnically determined, it explains the lower BMI at which South and East Asian populations develop diabetes, and why beta-cell dysfunction can be reversed once organ fat falls below the individual’s threshold. Supporting evidence comes from the DiRECT trial, in which a structured low-energy diet producing roughly 10 kg weight loss returned nearly half of participants to non-diabetic glycaemia, and from the observation that diabetes remits and relapses in line with crossing this fat threshold. Clinically it reframes obesity-related diabetes as a potentially reversible ectopic-fat disease rather than an inevitable progressive one, and underpins why weight targets should be personalised rather than set by a single BMI number.

Why It Matters

This is why we set weight goals around you, not around a generic chart — the amount of weight one person must lose to protect their pancreas can differ greatly from another’s. Understanding your personal threshold helps us aim for the loss that actually reverses diabetes risk, not just a number on a scale.
