Type 2 Diabetes Remission

Type 2 diabetes remission means blood sugar levels return to the non-diabetic range and stay there for at least three months without glucose-lowering medication.

Plain English

Patient-Friendly Explanation

Type 2 diabetes remission is the closest thing to reversing the diagnosis — but it is not a cure. Remission means your blood sugar returns to the non-diabetic range and stays there for at least three months without taking glucose-lowering medication. The strongest and most reliable driver of remission is substantial weight loss, usually around 10 to 15 percent of body weight, which reduces the fat stored in the liver and pancreas and allows them to work normally again. Remission is most achievable when diabetes is caught early, and it still requires lifelong monitoring because blood sugar can rise again if weight is regained.

Medical

Clinical Definition

Consensus definitions (ADA 2021) describe type 2 diabetes remission as HbA1c below 6.5% (48 mmol/mol) sustained for at least three months in the absence of glucose-lowering pharmacotherapy. The underlying mechanism is the reversal of ectopic fat accumulation in the liver and pancreas — the personal fat threshold concept — which restores hepatic insulin sensitivity and improves beta-cell function. In the DiRECT trial, a structured low-energy diet achieving mean weight loss of about 10 kg produced remission in nearly half of participants at 12 months, with greater weight loss predicting higher remission rates; metabolic surgery achieves the highest and most durable remission rates. Remission is not cure: relapse follows weight regain in many cases, and ongoing monitoring of glycaemia, renal function and retinopathy remains mandatory.

Why It Matters

For people diagnosed with type 2 diabetes in recent years, remission is now a realistic goal we can work towards together rather than a distant hope. Our programme focuses on achieving and protecting the double-digit weight loss that gives the best chance of putting diabetes into remission.
