Muscle Loss During Weight Loss

Muscle loss during weight loss is the unintentional loss of lean body mass that occurs when dieting, especially with rapid weight loss, low protein intake or no resistance exercise.

Patient-Friendly Explanation

When people lose weight, the scale does not distinguish fat from muscle — and a surprising share of the weight lost on aggressive diets is muscle, not fat. Losing muscle matters because muscle burns calories even at rest, supports mobility and strength, and helps keep weight off long term. The good news is that with a moderate calorie deficit, enough protein and resistance training, most of the weight lost can be fat rather than muscle.

Clinical Definition

During caloric restriction, lean mass can contribute 20–30% or more of total weight lost, particularly with very low-calorie diets, inadequate protein, or the absence of resistance training; pharmacotherapy and bariatric surgery vary in their lean-sparing effects. Sarcopenic obesity — low muscle mass relative to fat — is an independent risk factor for metabolic disease, falls and frailty. Preservation strategies include a modest deficit (0.5–1 kg/week), protein intake of 1.2–1.6 g/kg/day, progressive resistance training 2–3× weekly, and monitoring of body composition rather than weight alone.

Why It Matters

The number on the scale can lie — losing 5 kg of muscle is not the same achievement as losing 5 kg of fat. We track body composition in our programme so patients protect the tissue that keeps their metabolism healthy.
