Fibroblast growth factor 21, usually shortened to FGF21, is a hormone-like protein produced mainly by the liver that helps the body manage energy. It acts on many organs to improve blood sugar control, increase fat burning and support the switch to burning fat for fuel when food is scarce. In people with obesity, FGF21 levels are unusually high — the body seems to stop responding to it properly, a state called FGF21 resistance. Because of its powerful metabolic effects, medications that imitate FGF21 are being studied for fatty liver disease and other metabolic conditions.

Clinical Definition

FGF21 is a hepatokine (and, to a lesser extent, adipokine) that signals through FGF receptor 1c in complex with the co-receptor beta-Klotho to regulate glucose and lipid metabolism, energy expenditure and ketogenesis. Circulating FGF21 rises physiologically with fasting, ketogenic diets and cold exposure, but is chronically elevated in obesity, insulin resistance and non-alcoholic fatty liver disease, consistent with an FGF21-resistant state in which signalling is impaired despite high ligand concentrations. Experimental FGF21 analogues improve hepatic steatosis, dyslipidaemia and glycaemic control in clinical trials for metabolic dysfunction-associated steatotic liver disease, positioning this pathway as a therapeutic target. FGF21 is not yet part of routine clinical testing, but it illustrates how the liver acts as an endocrine organ in metabolic disease.
