Setmelanotide (Imcivree)

Setmelanotide (brand name Imcivree) is a prescription injection for people with specific genetic conditions that cause severe, unrelenting hunger and early-onset obesity. It works by activating a pathway in the brain that normally signals fullness after eating.

Setmelanotide is an MC4R (melanocortin-4 receptor) agonist approved for weight management in patients with obesity due to POMC, PCSK1, or LEPR deficiency confirmed by genetic testing, and for Bardet-Biedl syndrome. By directly activating the MC4 receptor downstream of the leptin signalling pathway, setmelanotide restores satiety signalling independent of leptin, leading to substantial weight loss (10% or more in approximately 80% of responders) in these rare monogenic obesity syndromes.
