Dulaglutide (Trulicity)

Dulaglutide, sold under the brand name Trulicity, is a once-weekly injection originally developed for type 2 diabetes. It works by mimicking a natural hormone called GLP-1 that slows stomach emptying, reduces appetite, and helps the pancreas release insulin. Many patients experience meaningful weight loss on Trulicity, even though weight loss is not its primary approved indication.

Dulaglutide is a once-weekly GLP-1 receptor agonist with a large fusion protein structure that extends its half-life to approximately 5 days, enabling once-weekly dosing. It activates GLP-1 receptors in the pancreas (enhancing glucose-dependent insulin secretion), gastrointestinal tract (delaying gastric emptying), and hypothalamus (promoting satiety). Clinical trials (AWARD programme, REWIND) demonstrate mean weight reductions of 2–4.5 kg, with dose-dependent efficacy up to the 4.5 mg weekly dose. It is approved for type 2 diabetes and cardiovascular risk reduction, with weight loss being an established off-target benefit.
