Setmelanotide is an MC4R (melanocortin-4 receptor) agonist approved for weight management in patients with obesity due to POMC, PCSK1, or LEPR deficiency confirmed by genetic testing, and for Bardet-Biedl syndrome. By directly activating the MC4 receptor downstream of the leptin signalling pathway, setmelanotide restores satiety signalling independent of leptin, leading to substantial weight loss (10% or more in approximately 80% of responders) in these rare monogenic obesity syndromes.
