How Is Wegovy Taken? The Once-Weekly Injection Explained

One of the most common questions patients ask is simply practical: how is this actually taken? Wegovy (semaglutide 2.4 mg) is a medicine given as a small injection under the skin, once a week, using a pre-filled pen. Here is what that means in practice — explained without the jargon.

The format: a once-weekly pen

The medicine comes in a pre-filled, single-use pen designed for home use:

• One small injection once a week — the same day each week
• The needle is very fine and not visible in normal use — most patients describe a brief, mild pinch at most
• The pen is used for a set number of weeks, then replaced with the next dose step

The once-weekly format is deliberate: the medicine is long-acting, so one dose sustains its effect across the week — and one weekly routine is easier to keep than a daily one.

The dose build-up

Treatment does not start at the full dose. The schedule follows a standard escalation — 0.25 mg for the first four weeks, then 0.5 mg, 1 mg and 1.7 mg at four-week intervals, reaching the 2.4 mg maintenance dose at about week 16. Each step uses a different pen strength, and your doctor may hold a step longer if you are sensitive to it.

This build-up exists for two reasons: to let your body adjust gradually (which keeps digestive side effects manageable) and to find the dose that works for you. The schedule is set by your doctor — it is not a do-it-yourself plan.

Injection sites and technique

The injection goes under the skin (not into muscle) of:

• The abdomen (avoiding the area around the navel)
• The thigh, or
• The upper arm

Sites can be rotated between weeks. At your first visit, the doctor or nurse demonstrates the technique and watches you do it once — most people are comfortable quickly. The pen is designed so the dose is delivered automatically with a press.

Storage and handling

The pen is stored in the refrigerator at 2–8°C until first use (never frozen), and after first use can be kept at room temperature (below 30°C) for up to six weeks, away from direct heat and sunlight. The medicine is not used if it has been frozen or if the pen looks damaged. Your clinic gives you the full storage instructions with your first pen.

Missed doses

If a dose is missed, the general approach is: if it’s only a few days late, take it as soon as you remember and return to your usual weekly schedule; if your next dose is close, skip the missed one and stay on schedule. Never take two doses to make up. Your doctor will give you the specific rule for your plan — and when unsure, a quick message to the clinic resolves it.

Why supervision still matters

Even though the injection is simple, the programme is the treatment: assessment before starting, a prescribed build-up, scheduled reviews of weight, measurements and side effects, and adjustment when needed.
