MC4R Agonist Therapy
An injectable treatment for severe obesity caused by specific rare genetic conditions affecting appetite regulation.
Plain English
Patient-Friendly Explanation
MC4R agonist therapy is a targeted medication for a rare form of obesity caused by a fault in a specific gene — the melanocortin-4 receptor pathway that controls appetite. It replaces the missing signal that tells the brain you are full. This treatment is for carefully selected patients, not general weight loss.
Medical
Clinical Definition
MC4R agonist therapy targets the melanocortin-4 receptor (MC4R) pathway, a critical hypothalamic regulator of energy balance and appetite. Loss-of-function variants in MC4R are the most common cause of monogenic obesity, found in roughly 2–5% of people with severe early-onset obesity; upstream defects in the POMC and leptin receptor pathways also cause MC4R pathway deficiency. Setmelanotide, a synthetic MC4R agonist, is approved for obesity due to biallelic POMC, PCSK1 or leptin receptor deficiency, producing substantial weight loss and reduced hunger in clinical trials. Genetic testing is required before treatment, and the therapy is not indicated for common polygenic obesity.

Why It Matters
This is precision medicine in action — a drug matched to a specific genetic cause of obesity. It is rare, but identifying it matters: these patients need the right diagnosis, not another generic diet.
